Hang Checks and loading up the Hang Straps

Yesterday at Jacks a pilot's main hang strap failed and his backup proved its worth. Apparently he did a step through and did not lay down to fully load up the main strap. The backup loop slid back 4 inches and made the glider tail heavy the whole flight. The pilot had to pull in as the pitch stability was greatly compromised.

Do you know how it failed (i.e. broke, wasn't connected properly)? Also what glider?

It wasn't connected properly. Whatever it was connected to gave way immediately upon loading up the main hang strap during the launch. This would have been discovered had the pilot done a proper static hang check and loaded the main hang strap prior to launching. The glider model was a TRX. Bacil
